The current eBook cover fee is a flat $67.00 per cover, which includes 5 cover sizes from large to small. Stock image fees are included unless you intend on using more than 5 stock images. Currently I use www.bigstockphoto.com for stock images, and may also use public domain sources when and where I can.
I’m usually good for unlimited revisions, unless things get completely out of hand – meaning that if you intend on changing the thing 50 million times right from the get go, it will be very time consuming for both of us. I’ll have to put a stop to “unlimited revisions” if it’s being abused. Otherwise, I have made changes to covers in my files two years after they are originally completed, and yes I keep backups.
If you lose your files in a computer crash – chances are that I’ve got them on disc – 5 years later. 
The $67.00 cover fee does NOT include print covers, due to associated stock cost, working with bleed edges, print sizes, etc. Please email me if you need an eBook cover that will be in print.
